Part 2 of the award application consists of submitting the following three items by email:
- Submit a Word document containing a written explanation of your organization’s current and future employee health efforts. Tell us how you are working to integrate the Areas of Expertise (health/medical plan, employee benefits, wellness, and workers comp/safety/risk management) and all of the things your company does to make it a Rock Star of Health! Click here for some ideas and questions to consider while writing about your health efforts.
- Submit an image of your company’s logo.
- Optional: To support your submission, include any other supporting documents, fliers, program announcements, reports, surveys, articles, announcements, etc.
IMPORTANT! Applications for Employer Rock Star of Health Awards are evaluated based only on the information submitted. Stating “refer to our website” or similar will be insufficient.
